NOTE: This form is needed by March 1 or as soon as your officers are elected.

Please provide the names of those currently holding an office your chapter. NALS will utilize its membership database to pull all contact information needed. In keeping with the new committee structure, please list all officers when possible. We will be asking for participation from your leadership on the various national committees.

This form should be completed and submitted online. If you wish to fax or mail this form please print this page and fill out the information.

It is imperative that NALS receive an officer roster in order for your chapter to receive important mailings, rebate checks, and new member contacts. Your NALS.org Manual will be sent to your President upon receipt of your submission of this form. (This first mailing will be after March 15.)
